HEALTHY ME WELLNESS TEAM SPONSORSHIP
Ellis Medicine employees and their spouses who participate in recreational sports leagues can apply for Healthy ME assistance. This financial assistance is up to $500 for qualified applicants.
Guidelines:
- Ellis Medicine employees must choose a Team Activity that enhances their well-being
- Examples include, but are not limited to soccer, volleyball, basketball, kickball, softball, etc (entry fees, admin costs, but does not include fund raising amounts, equipment, other fees).
- If you are unsure of what qualifies, please email wellness@ellismedicine.org
Steps:
- Follow the instructions on the application Process Document
- Have the team captain print and fill out the Team Sponsorship Application
- Print and have Ellis Medicine team members sign the Physical Activity Waiver form
- Once the application is completed and the Physical Activity Waiver is signed, email to wellness@ellismedicine.org, Fax to 518-347-5522 or mail to Ellis Medicine, Attention HR 3951, 600 McClellan St., Schenectady, NY 12304
