Don’t forget! The 2016 Passport deadline is TODAY – November 29!

The six Passport metrics are:

• 2015-2016 Influenza Vaccine – the 2016-2017 vaccine you receive this fall will count for next year’s Passport
• Preventive Physical Exam – if you received your physical between November 1-29, you must have your doctor complete the Health Practitioner section inside the Passport
• Personal Health Assessment – available online, view instructions to complete the PHA here
• Tobacco Free Attestation 
• Healthy MEE Activity 1 
• Healthy MEE Activity 2
